All of these requests are referred to the Human Resource Department. Personal Information may be provided in written format to an external party only when a form is provided by the requestor and the form is accompanied by an authorization for the release of such Personal Information signed by the current or former Syclo employee. Oral requests for Personal Information will be limited to confirming information stated by the external party. Syclo employees that participate in Syclo benefit plans should be aware that Personal Information will be shared with plan providers as required for their claims handling or record keeping needs.Syclo Employee Obligations Regarding Personal Information
All Syclo employees are obligated to treat the maintenance, access and disclosure of any Personal Information whether employee or non-employee Personal Information with the same degree of confidentiality and care as that required of Syclo towards its employee's Personal Information.Request for Personal data, Rectification and Destruction of Personal Information
All individuals on whom Syclo holds data have the right to request copies of their Personal Information for a small administration fee.If an individual believes his Personal Information is incorrect he should report this to the Human Resources Department and provide the evidence. Human Resources will correct factual information which it, in its sole opinion believes to be incorrect. In all other instances the original data and the individual's comment on the data will be recorded.
Syclo does not retain Personal Information (or Sensitive Personal Data) beyond the need required by its business purposes except as required by various governmental and regulatory agencies.
Violation of Privacy Policy Regarding Personal Information
Syclo employees should report to the Human Resources Department any violations of Syclo's Personal Information Privacy Policy. The Human Resource Department will investigate and take any corrective action deemed necessary after the investigation. A standard of reasonableness will apply in all circumstance regarding violations of this policy. The following list of reasonable forms of disclosure of Personal Information includes but is not limited to:- Release of partial employee birth dates, i.e., day and month is not considered confidential.
- Personal telephone numbers or e-mail addresses may be distributed to managers in order to facilitate work schedules and business operations.
- Employee identifier information used in salary or budget planning, review processes and for timekeeping purposes will be shared with managers.
- Employee and dependent information may be distributed in accordance with open enrollment processes for periodic benefit plan changes or periodic benefits statement updates.
